I hereby acknowledge that I have read the regulations governing The Basic Training Rodeo Queen Clinic; that my conduct as a participate of this clinic will be in accordance with the regulations established in the clinic rules and regulations. I acknowledge these regulations to prevail throughout all activities and functions; and said regulations pertain to all persons related to my appearance in the clinic.
I hereby grant permission to the Basic Training Rodeo Queen Clinic Director(s) to verify any information contained on my application.
I hereby release the Basic Rodeo Queen Clinic individuals associated with it, from responsibility of injury or loss to persons or property, involving me or any persons related to any appearance in the Clinic, and which applies also to the heirs, assigns, executors or administrators for me or persons relative to my appearance; and, that such release shall include all accredited sponsors, all officers, directors, and member individual and groups of the Basic Rodeo Queen Clinic, including the executives and committee. That I hereby release title, claim, and rights to photographs and/or public relations materials involving me for the use of clinic promotions.
The Basic Training Rodeo Queen Clinic will be released from any, and all, liability resulting from my action.
